Most breeders mean to follow up with placements but never get around to it. The result is no early warning when a placement is struggling, no record of how puppies turned out long-term, and no testimonials to show future buyers what your program produces.
The Post-Placement Check-In is the form you send at 30 days, 6 months, and 1 year after pickup. Same form each time, sent by email, takes the buyer five minutes. It serves three purposes at once: confirms the placement is going well, surfaces concerns early enough to help, and captures the photos and short testimonials that become program credibility for the next round of buyers.
What the document does
It walks the buyer through eight sections covering how things are going overall, the puppy's health and vet care, behavior and training progress, daily life, and any questions for you. The final sections invite testimonials and photos with explicit consent options, including how the buyer wants their name attributed (first name only, full name, or anonymous).
The form is designed to feel warm rather than bureaucratic. Buyers are more likely to respond to a 5-minute form than a 20-minute one, and the questions are framed as a check-in from someone who cares, not an audit.
