Litter Vet Visit Record
Use this to keep vet visit outcomes attached to the litter instead of scattered across invoices, texts, and memory.
Instant download · .xlsx + .pdf · Single-business use
By the time a litter goes home, each puppy has typically had two to three dewormings, one to two vaccinations, a vet exam, a fecal test, and a microchip. Track that in your head or in scattered notes, and at go-home day you will be reconstructing what happened to each puppy from memory. That is exactly the moment when a missed deworming becomes a problem.
The Litter Vet Visit Record gives you a single document tracking every treatment for every puppy in the litter. Each puppy is a row. Each treatment is a column. Cells fill in as treatments are completed. A summary row at the bottom shows you at a glance what is still outstanding across the whole litter.
What the document does
It centralizes the medical history of the entire litter so nothing slips through the cracks before placement. The structure means you can look at a single sheet and answer "did puppy 3 get their second deworming?" without flipping through notebooks. The completion summary at the bottom rolls up each treatment column, so you can see which treatments are done across all puppies and which still need to happen.
What is included
- Per-puppy rows for up to 12 puppies, with columns for each treatment category
- 9 treatment categories covering deworming rounds 1-4, vaccination rounds 1-2, fecal test, vet exam, and microchip
- Completion summary row at the bottom that totals completed treatments per category
- Per-puppy header capturing call name, sex, color, microchip number, and notes
- Notes column per puppy for individual flags (slow weight gain, vet observations, treatment reactions)
- Printable PDF version for breeders who prefer to record at the vet in the moment and transcribe later
Format and how it works
Includes an editable spreadsheet (.xlsx) with formulas for the completion summary plus a printable PDF version. The .xlsx works in Excel, Apple Numbers, and Google Sheets. Most breeders update the spreadsheet right after each vet visit while the details are fresh.
Use one file per litter. The data feeds directly into the per-puppy Health Records Transfer Form on go-home day, so each new owner leaves with a complete record of their puppy's medical history.
Why this version is different
Most vet records for a litter live in a notebook, a vet's printout, or a memory. This one keeps the whole litter in one place and visible at a glance. The completion summary catches the puppy that was missed for round 2 dewormings before it becomes a placement-day problem. Tracking microchip numbers and registration company in the same record means the chip data is right where you need it when filling out the Health Records Transfer Form.
Who this is for
For breeders managing multi-puppy litters who want a single source of truth for medical history. Especially valuable for breeders prepping for go-home day, since this record feeds directly into the per-puppy Health Records Transfer Form. Pairs with the Whelping Log, the Puppy Weight Tracker, and the Health Records Transfer Form.
