A completed title is one of the most significant outcomes a breeding program produces. Most breeders have the certificate in a frame or a folder somewhere and a general memory of when and where it happened. Ask them the judge's name, the certificate number, or which show finished the run, and most will have to look it up or guess.
The Championship Certificate Log is the permanent program record that answers all of those questions in one place. It is not a tool you use every show. It is the document you complete after a title is certified and keep for the life of your program.
What the document does
It runs the exhibitor through three sections that together create a complete archive of every title the program has finished. The title log is the quick reference, one row per title in chronological order. The milestones section captures the landmark moments above the point totals. The detail records section gives each significant title its own block with the full accounting of what happened.

- Section 1: Title Log covering all dogs and all titles in a single 16-row table with columns for title earned, registry, date completed, show where finished, finishing judge, certificate number, and notes
- Section 2: Program Milestones table for landmark wins above the standard title progression, first champion, first group placement, first Best in Show, or any win that meant something beyond the points
- Section 3: Six per-title detail records, each capturing dog identification, title earned, finishing show and judge, certificate number and storage location, a points and majors breakdown block, a dated free-write field for recording the moment the title was finished, and a photography and media record for the show
Format and how it works
Includes a Word document. Complete one detail record after each title is certified. The title log at the front is updated the same day the detail record is filled in. Store the document with your program records. The certificate number fields are filled in from the registry's official certificate when it arrives.
Why this version is different
Most programs have no permanent title record beyond the certificates themselves. When a certificate gets damaged, a program changes hands, or a buyer asks for a complete show history, the gap between "we finished this dog" and "here is the date, the judge, the show, and the certificate number" becomes expensive. The detail records section is also the one part of the kit that captures something the spreadsheets cannot, which is the story of how a title was actually finished and who was there.
