Document · Keeper's Emergency
Complete before the visit. Captures animal info, symptoms, husbandry, feeding, history, and recent changes so the vet does not spend the appointment extracting context from you.
About this document
The most expensive part of an exotic vet visit is the time the vet spends extracting information from a stressed owner. The Pre-Visit Worksheet replaces that extraction with a structured form the keeper fills at the kitchen table before leaving. Hand it to the vet tech at check-in. Expect the appointment to run 30 to 50 percent shorter and the diagnosis to be more accurate.
The worksheet asks every question the vet asks, in the order the vet asks them, so the keeper arrives ready instead of remembering details mid-appointment.
What the document does:
It captures six sections of pre-visit information. Animal information (name, species, age, weights, source). Why you are here today (main symptom in one sentence, when first noticed, trend, other symptoms). Husbandry snapshot (temps, humidity, UVB, substrate, enclosure size). Feeding and supplementation (food, frequency, last meal, calcium and multivitamin schedule). Health history (last defecation, last shed, previous vet visits, current medications). What changed recently (any change in the 30 days before symptoms appeared).
What is included:
Format and how it works:
Word version (.docx) for editing and a PDF version for printing. Most keepers print the worksheet, fill it by hand at the kitchen table, and hand the physical copy to the vet tech at check-in. Some keepers fill it digitally and email a copy ahead of the appointment.
Why this version is different:
Most exotic vet visits open with the vet asking the same 25 questions the keeper has to remember from scratch. The Pre-Visit Worksheet eliminates that bottleneck. The "what changed" section is the highest-yield because the answer almost always points to the husbandry root cause, and keepers rarely think to mention it without prompting.
Who this is for:
For gecko keepers facing an upcoming vet visit and for breeders who want every buyer to walk into the exotic vet prepared. Pairs with the Vet Now Checklist (which the keeper completes in parallel before transport), the Post-Visit Follow-up Log, and the Daily Husbandry Log (which feeds the husbandry snapshot section).

Updates
You bought version 1. When a state adds a registry requirement, a new federal rule lands, or a working breeder catches something that belongs in the document, the new file shows up in your inbox. No upsell, no annual subscription, no "unlock by upgrading." The document is yours, updates included, forever.
Common questions
The contracts were drafted with an attorney and pressure-tested across multiple working programs, but they are templates — not a substitute for a lawyer for your specific state's laws. Each file ships with a clear note about that. Most breeders use them as a starting point and have a local attorney review for state-specific edits.
DOCX + PDF. The DOCX opens in Word, Google Docs, Pages, or LibreOffice. The PDF is fill-on-screen or print-and-fill. Everything is editable.
When I ship a new version of this document, the new file lands in your inbox. Laws shift. A state adds a registry requirement. A working breeder tells me something that belongs in the document. The next version goes out to everyone who bought the previous one. No upsell, no annual subscription.
14-day no-questions-asked refund. If the documentdoesn't fit your operation, you get your money back and I'd genuinely like to hear why so the next version is better.
Yes, by a wide margin if you need more than two or three documents. The Keeper's Emergency Kit bundles this and every other keeper's emergency document for $18.50.
